The Oxford County Community Health Centre serves individuals who are vulnerable and/or marginalized, with complex conditions, experiencing barriers to accessing care. We are seeking a permanent, full-time (5 days per week, 37.5 hours):

REGISTERED NURSE – CHRONIC DISEASE MANAGEMENT (CDM) & RAPID ACCESS ADDICTION MEDICINE (RAAM) CLINIC

Overview of Position: We are seeking a compassionate and experienced Registered Nurse (RN) to lead our Chronic Disease Management program and support our RAAM program. This dual role involves providing care to our rostered clients living with chronic diseases such as Diabetes and COPD. Additionally, the role expects direct involvement in our RAAM clinic which services clients living with substance use disorders, primarily Alcohol Use Disorder and Opioid Use Disorder. You will work collaboratively with our multidisciplinary team, offering clinical assessment, pharmacological and therapeutic guidance, as well as group education programming.

REQUIREMENTS:

  • Current Registration as an RN and a member in good standing with the CNO.

Preference will be given to candidates with:

  • Demonstrated experience and comfort-level working with vulnerable populations.
  • Demonstrated experience and comfort level working with those with Chronic Disease such as Diabetes and COPD. (Certified Diabetes Educator certification or willing to work towards obtaining would be required).
  • Satisfactory completion of a CNO approved RN prescribing course required
  • Demonstrated experience and comfort level working with patients who use substances.
  • Demonstrated ability to work independently and collaboratively within an interprofessional team.
  • Community experience, preferably in primary health care or combination of community, hospital and/or public health.
  • Strong communication & interpersonal skills, combined with excellent critical thinking, problem solving and organizational skills.
  • Strong aptitude for “meeting clients where they are at” – assisting clients in directing their own journey.
  • Strong computer skills including previous experience with an Electronic Health Record (PS Suite an asset).

Responsibilities
  • Provide evidence-based care to patients with multiple chronic disease such as Diabetes and COPD.
  • Apply clinical expertise and knowledge around therapeutic and pharmacological management of Diabetes and COPD.
  • Provide yearly COPD action plans for patients to treat exacerbations (with consultation of patient’s MRP).
  • Create and run group programming for patients and community members living with chronic disease.
  • Work collaboratively with our Community Paramedicine (CP) program to support patients living in the community with chronic disease, and avoid ER visits.
  • In our RAAM clinic you will provide nursing support to our clients living with substance use disorders and provide intake assessments, follow-ups, and administer injectable opioid replacement therapy.
  • Collaborate effectively with interdisciplinary team members – including nursing, physicians, Nurse Practitioners, outreach, social work, and coordinate referrals to appropriate internal and external supports as needed.
  • Document assessments, treatment plans, and clinical progress accurately and in a timely manner.

    Location: OCCHC Woodstock Site, with on-site collaboration at our Tillsonburg site as needed.
