You will provide primary care, chronic disease management, and health education, empowering patients to take an active role in their wellbeing while working collaboratively with correctional staff to deliver holistic, culturally sensitive care. Role – Registered Nurse (Chronic Disease) As a Registered Nurse – Chronic Disease, you will identify, manage, and support patients with chronic conditions, conduct assessments, develop care plans, coordinate referrals, and deliver health education. You will work closely with a multidisciplinary team to ensure evidence-based and culturally appropriate care. Requirements: Current AHPRA registration Minimum 2 years post-graduate experience (ideally in chronic disease, primary health, or correctional health) - post-graduate qualifications desirable and Willingness to complete a comprehensive probity assessment including a National Police Check. ALS, BLS. Vaccination Evidence Willingness to undergo a pre-employment medical and psychometric test. Why Serco?
