Position Overview: We are seeking either 1 full-time or 2 part-time compassionate and driven Registered Nurses (RNs) to support our clients in the home setting. This role is ideal for someone who is energized by building relationships, thinking critically, and taking initiative to improve outcomes—especially for individuals living with dementia and Parkinson’s disease. Key Responsibilities: Conducting in-home clinical assessments to evaluate health status, safety, and overall well-being Developing and overseeing individualized care plans that promote stability, safety, and quality of life Identifying and mitigating risks within the home to help prevent decline and avoidable hospitalizations Managing and/or administering medications, providing wound care and other skilled nursing tasks in accordance with physician orders, as applicable. Providing client advocacy and initiating referrals to other service providers per needs upon assessment. Providing clinical support and guidance for clients and their families Directly supervising, training, and supporting Certified Home Health Aides (CHHAs) to ensure consistent, high-quality care Maintaining strong continuity of care through follow-up visits, communication, and coordination with families and care teams Documenting care accurately and efficiently using EMR systems Qualifications: Active Registered Nurse (RN) license in good standing Experience in home care, geriatrics, or chronic/neurological conditions preferred Strong clinical judgment with the ability to translate assessments into effective care plans Comfortable and proficient with laptops/tablets, EMR systems, and G Suite (Google Workspace) Excellent interpersonal and client engagement skills Self-directed, dependable, and motivated to contribute to a growing organization Willingness to travel throughout our service area, including Somerset County and northern Middlesex County, with reliable transportation Work Expectations & Schedule: Immediate availability and flexibility 15 - 20 hours per week primarily weekdays with flexibility for mornings/days and afternoons/evenings, with weekends as needed Schedule is coordinated in advance with leadership to ensure client coverage and responsiveness RN is expected to maintain an up-to-date shared calendar of office and field hours Work is performed in the field and/or office; remote work is limited.
