  • 3 days each week in a fast-paced clinical setting, providing frontline care, supporting our GPs, and making sure our clients get the help they need when they need it.
  • 2 days each week dedicated to chronic disease management, working closely with clients to plan, educate and empower them to take control of their health and wellbeing.

It is a role where you will balance hands-on nursing with longer-term, relationship-based care, meaning you’ll see the immediate difference you make, as well as the lasting impact over time.

Responsibilities

ABOUT THE ROLE

You will be part of a supportive, passionate team delivering culturally safe care to our community.

In this role, you will:

  • Manage urgent clinical needs, coordinate GP appointments, and keep our clinic running smoothly.
  • Provide clinical care including wound management, diagnostic procedures, immunisations, health assessments and care planning.
  • Deliver chronic disease services, including medication management, health checks, and case conferencing with GPs and allied health providers.
  • Support health promotion and preventative care programs to reduce illness and improve wellbeing.
  • Mentor Aboriginal Health Workers and contribute to high clinical standards across the service.
  • Advocate for clients and walk beside them on their health journey.
