As a Registered Nurse/Midwife (Community Health Nurse) with Country Health Connect, you’ll be at the heart of patient care, assessing needs and planning tailored care solutions for individuals in a community health setting. You’ll gain increasing autonomy, while working within a supportive multidisciplinary team to deliver high-quality, holistic care. This role offers 1 day per week, and you’ll have the flexibility to choose which day works best for you and the opportunity to backfill leave when required.

Key Responsibilities:

  • Provide nursing/midwifery care and case management to patients/clients, with increasing autonomy over time.
  • Plan and coordinate services with other disciplines and agencies, ensuring holistic care delivery.
  • Promote patient safety, risk minimisation, and safe work practices in line with quality improvement activities.
  • Educate patients, carers, and groups on health promotion strategies to improve outcomes.
  • Support the development of less experienced staff and students through mentorship and preceptorship.

What We’re Looking For:

  • Registered Nurse or Nurse/Midwife with current registration or eligibility with the Nursing and Midwifery Board of Australia.
  • Strong communication, problem-solving, and conflict resolution skills.
  • Ability to work within a multidisciplinary team, providing creative and flexible solutions to healthcare challenges.
  • Experience in delivering consumer-centred care and demonstrated ability to prioritise workload effectively.
  • Knowledge of relevant legislation, healthcare standards, and quality improvement systems.
