Guided by our values of compassion, excellence, and courage, and through our strategic roadmap, Plan to 2030 [https://strategy.thp.ca/], we are creating a new kind of health care - defined not by illness, but by the health and well-being of people and communities.     Position Summary: Trillium Health Partners (THP) operates a 39 bed post-acute medically Complex Continuing Care (CCC) unit at our Queensway Health Centre (QHC) location and is recruiting an individual who is highly skilled, agile and adaptable with advanced critical thinking and interprofessional skills.  As a member of the CCC, QHC Interprofessional team you will have the opportunity to provide care to the medically complex continuing care patient population.  Responsibilities: * Responsible and accountable for decisions and actions in the provision and management of Nursing care for patients in accordance with the standards of nursing practice and best practice guidelines * Assess and classify physical and emotional condition of patient through reports from staff and interviews with patient and family * Responsible for the provision of nursing care, teaching and coaching to patients and their families   Key Qualifications: * Current registration with the College of Nurses of Ontario * In good standing with the College of Nurses of Ontario * Current BCLS Certificate * Ability to perform phlebotomies, tracheostomy care and IV starts * Reliable and flexible individual to work as part of an interprofessional team * Experience in Complex Continuing Care, Gerontology and Long Term Care * Ability to think critically and effectively in order to prioritize needs * Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written * Positive attendance and work record essential   Internal candidates who believe they possess the necessary qualifications and experience for this position and who have been in their current position for at least six (6) months are encouraged to apply.
