Delivers patient centred care that coordinates individualised care from admission and throughout hospitalisation, incorporating discharge planning in the plan of care. Initiates referrals and utilises available resources for the purpose of comprehensive care to patients and families. Occasional trunk flexion greater than 15 degrees (eg. when changing dressings, administering medication at the bed side, making beds and completing personal care). Occasional pushing / pulling (eg. hoist, beds, observation machines, linen skip, dressing trolley and mobile shower chair). Rare crouching / squatting (eg. when assisting with lower limb cleaning or accessing items stored at ground level). PPE: Gloves, Waterproof Gown, Eye Protection.
