POSITION SUMMARY

Reporting to the Manager of Dialysis, Diabetes Education, OPNC and Paediatrics, the successful applicant will be responsible for the provision of Diabetes education to the patient in the Diabetes Education program. This will include assessment of patient’s educational needs, implementation of the education in the most appropriate format and evaluation of the effectiveness of the education. The successful applicant will work as a member of the Diabetes education team.

MANDATORY QUALIFICATIONS

  • At least 2 years recent experience working in Diabetes Education Clinic setting
  • Graduate of a University Nursing Program (BScN)
  • Current Basic Cardiac Life Support (BCLS) certification
  • Registration (in good standing) with the College of Nurses of Ontario

EDUCATION REQUIREMENTS

  • Certification in Diabetes Education preferred or actively pursuing Certified Diabetes Educator (CDE) Designation
  • Certification in Insulin Pumps or actively pursuing
