How you’ll make an impact in this role Drive Personalized Care: Play a crucial, hands-on role in the success of individualized patient care plans through active implementation and continuous monitoring of patient progress. Be the Clinical Compass: Serve as the team’s eyes and ears, vigilantly monitoring, accurately documenting, and promptly communicating any changes in a patient's condition to ensure timely and appropriate interventions. Execute with Precision: Meticulously receive, note, and carry out physician and nursing orders, guaranteeing the accurate and effective delivery of all directed treatments and care procedures. Lead Care Orchestration: Step into the role of primary coordinator, serving as the central hub of communication to seamlessly integrate and align all clinical disciplines for a truly holistic and well-orchestrated patient care experience. Facilitate Safe Transitions: Lead the patient discharge planning process by assessing ongoing needs and collaboratively coordinating strategic plans with the entire healthcare team, ensuring a safe, smooth, and supported transition to the next stage of care. What minimum requirements you’ll need Licensure / Certification / Registration: Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required. One or more of the following required: Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted. BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted. Pediatric Advanced Life Support preferred. American Heart Association or American Red Cross accepted. Education: Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.
