The Opportunity
We are seeking expressions of interest from experienced and passionate Community Registered Nurses to join our growing team in Western Sydney. This is an exciting opportunity to make a real difference in the lives of individuals by delivering high-quality, client-centred care in the community.

Key Responsibilities

  • Conduct comprehensive client assessments to identify health, mental health, and support needs.
  • Develop and review individualised care plans in collaboration with clients, families, and multidisciplinary teams.
  • Ensure clinical compliance with relevant legislation, professional standards, and organisational policies.
  • Monitor and evaluate client progress, escalating concerns as needed.
  • Provide clinical guidance and support to care staff and carers.
  • Promote health, safety, and wellbeing through education, advocacy, and empowerment.
  • Maintain accurate and timely documentation in line with regulatory and organisational requirements.

About You

  • Experience in community nursing, aged care, disability, or mental health.
  • Strong assessment, care planning, and clinical compliance skills.
  • Excellent communication and interpersonal abilities with a compassionate approach.
  • Ability to work independently as well as part of a multidisciplinary team.
  • Ability to learn and adapt in fast pace Environment.

Requirements

  • Resume
  • A minimum of 2 years experience working in the community
  • Current AHPRA registration as a Registered Nurse.
  • First Aid Certificate
  • A current police check
  • NDIS orientation certificate
  • COVID-19 infection control
  • Valid NSW driver’s licence and willingness to travel within Western Sydney.
  • NDIS Worker Screening Check (or willingness to obtain).
  • WWCC - working with children check (or willingness to obtain).
  • 100 points of ID (Passport, Drivers License, Medicare etc)
