ABOUT THE OPPORTUNITY
We are seeking a dedicated Registered Nurse to join our Community Health team. This role focuses on independent case management as a Care Coordinator, providing personalised care to clients in their homes.
You will collaborate closely with the Palliative Care, HITH, and Wound Care teams to ensure comprehensive patient management. The position offers flexible morning and afternoon shifts on a 7-day roster, including weekends and public holidays.
