Description

As a member of the Foothills Country Hospice Team, this individual – an experienced Registered Nurse, preferably with palliative experience, is responsible for providing and advocating for safe, compassionate, competent, ethical and evidence informed palliative and end of life care (As per CHPCA statement). The Registered Nurse will work in a team environment and will coordinate care in accordance with established policies, procedures, standard and resident safety.

REQUIREMENTS:

  • Current CRNA registration.
  • Current BLS Health Care Provider CPR.
  • Knowledgeable about, and committed to, the Hospice’s Mission, Vision and Values.
  • Preference will be given to individuals with palliative care nursing experience and training.
  • Must be willing to participate in educational advancement within the palliative care field.
  • Availability to participate actively in team meetings and quality improvement activities.
  • Awareness of one’s own limitations and strengths in emotionally sensitive situations and ability to react calmly and effectively.
  • Ability to represent the Hospice in an appropriate manner.
  • Ability to be flexible, work well in a new and changing environment and promote progressive/innovative thinking as well as team-based care.
  • Have knowledge of, and be sensitive to religious, ethnic and cultural diversity.
  • Must have good communication skills both verbally and non-verbally, and the ability to deal with people in crisis in a mature and non-judgmental manner.
  • Must have the ability to maintain a feeling of peace and comfort in the Hospice.
  • Ability to recognize the need for self-care in the Hospice workplace.

Responsibilities
  • Providing direct resident care in a team environment and demonstrating excellent pain and symptom assessment skills with timely management within the Registered Nurse (RN) scope of practice.
  • Consulting and collaborating with members of the care giving team such as; Medical Director, Physicians, Pharmacists, Clinical Manager, Licensed Practical Nurses (LPN), Health Care Aides (HCA), Social Worker, Volunteer Coordinator and Volunteers in order to provide safe, high quality hospice palliative care for residents and their families.
  • Coordinating care through working collaboratively with the LPNs, HCAs, volunteers and other members of the interdisciplinary team to provide care consistent with the Hospice’s Mission, Vision and Values.
  • Participating in rounds with multidisciplinary team members.
  • Working collaboratively with the resident, family, and care giving team to develop, implement, and evaluate the resident and family care plan.
  • Providing ongoing resident assessment and evaluation of physical, psycho-social and spiritual needs.
  • Documenting relevant information in a timely and accurate manner.
  • Being a positive role model for other employees, volunteers, residents and families.
  • Participating in bereavement support for families.
  • Completing required learning modules and competencies.
  • Assuming other nursing related duties as assigned by the Clinical Manager.
