QUALIFICATIONS, SKILLS AND EXPERIENCE:

  • Bachelor of Science in Nursing.
  • Minimum of 2 years of clinical experience in working with clients in community-based health/primary care settings.
  • Registered and in good standing with the College of Nurses of Ontario as a Registered Nurse.
  • Evidence of Professional Liability Insurance.
  • Experience working with people living with mental health challenges and/or substance use challenges.
  • Experience working with people who are unhoused is preferred.
  • Demonstrates a commitment to an anti-oppression model of care and harm reduction principles.
  • Knowledge and proficiency in current nursing interventions.
  • Knowledge of crisis intervention and conflict resolution.
  • Understanding of various chronic diseases such as diabetes, COPD, and heart disease.
  • Demonstrates the ability to work professionally and collaboratively in an interdisciplinary team environment.
  • Proficiency in the use of computers and software, particularly in Microsoft Word, Excel, Outlook, and electronic medical records.
  • Current CPR certificate.

Responsibilities

PURPOSE OF THE POSITION:

The Inner-City Family Health Team is a multidisciplinary team providing health services to unhoused or marginally housed clients. The successful candidate will work collaboratively with other RNs, physicians, the Substance Use Hub team and allied staff to provide comprehensive primary care services, including assessment, primary nursing care, case coordination, health education and healthcare management to clients experiencing chronic diseases, complex multi-system diseases, mental health and/or substance use.
The successful candidate will work Monday to Friday at our clinic at 69 Queen Street East and through community outreach. We seek an RN who works within a client-centred, trauma-informed, and anti-oppression model of care.

GENERAL RESPONSIBILITIES:

  • Sees clients for scheduled in-person or phone appointments and urgent walk-ins.
  • Provides nursing care within a primary care setting including health assessments, triaging, treatment, and health promotion.
  • Client care activities include but are not limited to: physical and psychosocial assessments, chronic disease management support, smoking cessation support, mental health interventions, harm reduction support, immunizations, TB skin tests, injectable medications, wound care, suture removal, ear syringing, phlebotomy, pap smears, and assisting physicians with minor procedures.
  • Engages clients in risk factor and health problem identification, goal setting and making choices of intervention for disease treatment, prevention and health promotion.
  • Works with the ICFHT team, the Substance Use Hub team and community organizations.
  • Maintains complete and accurate medical records for all clients.
  • Assists with managing and ordering medical supplies.
  • Participates in quality improvement activities.
  • Support client groups as needed.
  • Related duties as assigned.
