Job Summary We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Genesee County. The RN will provide skilled nursing care, coordinate treatment plans, and educate patients and families on managing health conditions safely at home. Competitive per-visit pay ($100 SOC / $65 RV), negotiable based on experience and coverage area Flexible part-time scheduling with local visits close to home Supportive administrative and clinical team to help you succeed Opportunities for growth in a respected and expanding home health agency Work with a compassionate team that values your expertise and care Responsibilities Conduct comprehensive Start of Care (SOC), Revisit, and Discharge assessments in patients’ homes Develop individualized care plans in collaboration with physicians and interdisciplinary teams Administer medications, perform wound care, and monitor vital signs Provide patient and caregiver education for condition management and recovery Document all visits, interventions, and progress in EMR promptly and accurately Coordinate with therapists, aides, and case managers to ensure continuity of care Maintain compliance with all state, federal, and agency policies Qualifications Qualifications Active and unrestricted Registered Nurse (RN) license in the state of Michigan Minimum 1 year of recent experience in Home Health preferred Strong clinical assessment, time management, and communication skills Reliable transportation and valid driver’s license required Compassionate, patient-centered approach to care Familiarity with EMR systems (Axxess or Home Health Notify preferred) Background Screening Safe Hands Home Health Care & Hospice conducts background screenings on all candidates through the Michigan LARA (Licensing and Regulatory Affairs) system upon acceptance of a contingent job offer.
