About the opportunity Conduct comprehensive assessments of elderly patients to develop and implement individualized care plans, ensuring optimal health outcomes. Administer medications accurately and monitor for adverse reactions, maintaining detailed records in compliance with healthcare regulations. Work closely with multidisciplinary teams, including physicians and allied health professionals, to coordinate patient care and address complex health needs Provide education and emotional support to patients' families Ensure adherence to aged care standards and regulations by maintaining accurate and timely patient records. Respond promptly to medical emergencies, utilizing advanced clinical skills to stabilize patients and initiate appropriate interventions.
