Description
Reports to the Unit Manager and Clinical Leader of the HHS Virtual Nursing-CHF and COPD program. The Registered Nurse working in Virtual Nursing Station- program-provides patient care in accordance with the telepractice standards of The College of Nurses of Ontario, and HHS standards, policies and procedures. The Registered Nurse will collaborate with an interdisciplinary team and will be responsible for all preparation required to conduct virtual visits and follow up with all patients receiving remote symptom monitoring including teaching self-management skills, scheduling follow-up appointments and following up with them by video or phone. Ambassador of the virtual program, and visiting patients & families in hospital to provide program information. 1. Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse 2. Recent (5-7 years) relevant clinical practice in medical surgical nursing 3. Experience working with hospitalized patients including seniors with multiple chronic conditions including COPD and CHF 4. Experience working with patients who are frail 5. Experience supporting patients to develop self-management skills 6. Experience identifying barriers and opportunities to support patients impacted by poor literacy, health literacy and social determinants of health 7. Knowledge of Ontario’s Chronic Disease Management Framework 8. Demonstrated leadership and management skills 9. Demonstrated excellence in communication, team-building and the ability to initiate and model positive change 10. Ability to communicate (written and verbal) effectively with patients, families, and other members of the healthcare team 11. Demonstrated effective interpersonal, public relations and problem solving skills 12. Ability to set priorities and organize work accordingly 13. Ability to deal with various hospital departments effectively and collaboratively 14. Membership in good standing with the College of Nurses of Ontario 15. Previous experience and knowledge of virtual care preferred 16. Experience with EPIC workflows and EMR best practices.
Responsibilities
The Registered Nurse will provide patient care in accordance with telepractice standards, collaborating with an interdisciplinary team to conduct virtual visits and manage remote symptom monitoring for patients in the CHF and COPD program. Responsibilities also include teaching self-management skills, scheduling follow-up appointments, and acting as a program ambassador by visiting patients and families in the hospital.
