Description
Reporting directly to the Clinical Manager, the Registered Nurse is responsible for facilitating the coordination of care of autologous and allogeneic bone marrow/stem cell transplant recipients and donors, from initial referral for transplant through to transplantation. This includes patient assessment, the coordination of pre-transplant diagnostics for recipients and donors in accordance with transplant timelines, donor searching and health screening, evaluation and follow-up of nursing interventions, and promoting the continuity of care across care settings and between health providers. The Registered Nurse functions as an advocate and primary contact for the patient/family, providing education, information and resources about transplantation, helping them to navigate the health system, and regularly communicating plans with the interdisciplinary team and related services throughout the pre-transplant trajectory of care. 1. Current certificate of registration with the College of Nurses of Ontario as a Registered Nurse 2. Current CON( c ) certification or equivalent advanced oncology nursing credentials (McMaster University Oncology Nursing Certificate, Mohawk College Oncology Certificate or equivalent from an accredited educational institution) 3. Current successful Completion of the DeSouza Standardized Chemotherapy and Biotherapy Course or Oncology Nursing Society (ONS) Chemotherapy/Biotherapy Certificate equivalent course Required. 4. Current BCLS certification 5. Minimum 3 years recent (within 5 years) Oncology nursing experience in the care of hematology patients receiving autologous and allogeneic stem cell transplantation 6. Minimum 3 years recent (within 5 years) experience in bone marrow/stem cell transplant coordination, including donor searching 7. Demonstrated knowledge of the Foundation for the Accreditation of Cellular Therapy (FACT) standards and Health Canada regulations related to stem cell transplantation 8. Demonstrated ability to work with Meditech, Clinical Connect, Sovera 9. Demonstrated ability to work independently and collaboratively within the interdisciplinary team 10. Demonstrated time management and critical thinking skills 11. Demonstrated interpersonal, communication, and patient teaching/counseling skills 12. Demonstrated commitment to the mission, vision and values of HHS
