Home Care is an interesting, challenging and continually changing field with some of the most complex clients in health care. AHS has an exciting opportunity to challenge your skills as an RN Case Manager within the thriving community of Fort McMurray. The Home Care Registered Nurse Case Manager provides case management services and interventions to clients with a variety of healthcare needs within Fort McMurray and outlying areas, including case management services to Willow Square Continuing Care Center. Case Manager’s are responsible for the assessment, planning, implementation, and evaluation of client centered care, in the setting most appropriate for the client. As a case manager, you will be central to the oversite and coordination of the healthcare needs of clients across the lifespan, including chronic disease management, assessment for assistance with ADL’s, wound assessments and treatments, assessments for placement in DLO facilities, supervision and assignment of care to Health Care Aides, as well as palliative and end of life care. AHS respects and supports client choices and collaborates with individuals, caregivers, professionals, and other community resources to optimize healthy living and connectedness within the community. In addition to a competitive rate of pay, AHS currently has a Fort McMurray Allowance in place to an annual maximum of $12,480. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
