Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. This is a part time/per diem position. Minimum 1 year Home Health experience required. Benefits Competitive salary Focused one to one patient care Flexible work schedule & reasonable work hours 401k matching Supportive and friendly work environment Requirements Minimum of 1 year of experience as a Registered Nurse with a valid license required Current CPR/BLS Certification Valid drivers license and automobile insurance as required by state law Comfortable driving to patient homes for patient visits Prior experience with Palliative Care preferred Prior experience in Home Health, Hospice, Infusion, or Home Care preferred Responsibilities Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient, physician, and other team members when appropriate. Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services. Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s home or living facility. Communicate necessary revisions to patient status and/or needs with the entire care team Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning. Provide complete, accurate and timely documentation consistent with the plan of care
Responsibilities
The Registered Nurse will initiate, develop, and implement the patient Plan of Care, revising it as needed in consultation with the care team. They are responsible for delivering quality, patient-centered care in the patient's home and ensuring accurate documentation.
