Cheer Home Care is looking for a Registered Nurse who loves spending time with older adults and provide high quality care in home. The RN collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses. . This position responsibilities will include: Complete assessment of client and needs Education to client and families Chronic Care Management Develop care plan and establish goals Post-surgical and wound care as needed Wellness support Current, unrestricted California registered nurse license 3 years of experience as a registered nurse Home Health experience preferred Proof of Negative TB result Physical Screening Documentation within the last year Active California Drivers license and car insurance Cell phone with Internet Access Fun Work Environment Supportive Team and Management Anthem Blue Cross or Kaiser Health Insurance United Concordia Dental Insurance Vision Insurance Matching 4% 401K Plan Career Advancement Opportunities Hourly Pay Rate: $48-$75 based on experience.
