Responsibilities: · Plans, coordinates, implements, and evaluates the nursing care of all patients. · Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs · Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients. Requirements A valid non-restricted RN license in the state of Colorado is required. Must have a valid non-restricted driver’s license. Flexibility and availability to take call for home health and hospice patients. At least 1 year of home health or hospice experience preferred.
Responsibilities
The nurse plans, implements, and evaluates nursing care for all patients while identifying specific needs and coordinating referrals. They work closely with an interdisciplinary team to provide comprehensive and responsive care plans.
