JOB DESCRIPTION

Seeking an experienced Registered Nurse to service home infusion patients in Grand Junction, CO and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will be responsible for educating patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote the patient’s well-being by providing high-quality nursing care.

(RN) REGISTERED NURSE REQUIREMENTS

  • Strong IV placement skills
  • Skilled in accessing Implanted Ports; Central Lines; PICC lines
  • Strong organizational and communication skills
  • Associate or Bachelor Degree in Nursing
  • Minimum of five years of nursing experience preferred in an Acute Care setting
  • Strong Med/Surg, ICU, ER, acute experience
  • Home Health/Infusion experience (preferred, not required)
  • Current CPR/Basic LIfe Support certifications (BLS) required
  • Background check required
  • Drug screen required
  • Malpractice insurance required

Responsibilities
  • Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System.
  • Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
  • Administer ongoing care/infusions for each patient, provide necessary follow-up as directed by the Physician and Clinical Manager.
  • Confer with physicians and pharmacies in developing the initial plan of treatment based on physician’s orders and initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician based on ongoing assessments and as required by policy/regulation.
  • Report patient care/conditions/progress to patient’s physician/pharmacy and Clinical Manager on a continuous basis.
  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
  • Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.
