Job Description As a community-based Registered Nurse, you'll travel to clients' homes across Southwest Victoria, on a part-time basis working Mondays, Tuesdays and Thursdays, providing personalised care and clinical expertise. You’ll be instrumental in improving quality of life, promoting independence, and delivering health outcomes that matter. In this rewarding position, you will: Deliver clinical nursing care including assessments, care planning and ongoing reviews Performing a wide range of clinical assessments (Falls Risk, Cognitive, Continence, Pain, Skin Integrity, Wound care, Medication Management Capacity, Carer Strain Index, etc.) Collaborate with the Client Care Manager (CCM) around the care and supports provided to enhance home care client outcomes Provide wound care and assisting with continence management and catheter care Monitor service quality, ensuring care aligns with regulatory and organisational standards Build strong relationships with customers, carers and key healthcare stakeholders Let’s talk about you! You are a caring and capable Registered Nurse with a strong commitment to delivering high-quality care in a community setting. You bring sound clinical knowledge, excellent communication skills and the ability to work both independently and collaboratively. You ideally have palliative care and dementia experience, a proactive mindset, confident managing clinical responsibilities, engaging with stakeholders and supporting positive outcomes for older people. Key requirements include: Bachelor of Nursing and current AHPRA registration (unrestricted) Minimum 5 years’ post-graduate clinical experience (ideally in aged care or community care) Strong understanding of aged care standards, WHS and risk management practices Current First Aid Certificate (HLTAID011) and Basic Life Support (HLTAID010) including Provide Cardiopulmonary Resuscitation (HLTAID009) National Police Check suitable for aged care Full driver’s licence and your own vehicle If you’re looking to take the next step in your nursing career and be part of a leading healthcare provider, we’d love to hear from you.
