Seeking flexible team members to be an integral part of our innovative and growing Hospital at Home program. Our Registered Nurse Field position is a full-time position with weekend rotation—Lake Forest Hospital Campus, with fleet vehicle provided for field coverage.

The Registered Nurse embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under the direction of the department leadership, consistently reflects the mission, vision, and values of NM, adheres to the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Home Hospital Nurse treats acutely ill patients in the home as a substitute for in-hospital care. The Home Hospital Nurse is a forward-thinking, creative individual who wants to practice in an innovative clinical setting and values interdisciplinary teamwork and patient-centered care. The Nurse will provide hospital-level acute care in a patient’s home and will be part of the NM Hospital at Home team including (depending on each patient’s needs) Physicians, Paramedics, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Home Health Aides, Physical Therapists, and other operational staff. Provides home visits within defined radius around site hospitals according to Hospital at Home care Model
