JOB DESCRIPTION

The Home Care Services Program (HCSP) is a Medicaid funded program that provides long term care to medically fragile and vulnerable individuals who require assistance with activities of daily living through the Community Alternative Systems Agency (CASA), Managed Long Term Care (MLTC) Program, the Assisted Living Program, Care at Home Program and Homebound Medicaid. The Medicaid Eligibility Unit is responsible for the Medicaid eligibility review and processing of initial applications, change actions, and renewals for all Medicaid funded long term care programs. This unit is responsible for the processing of Medicaid eligibility for consumers who receive services through one of the HCSP programs.
Under general supervision, with wide latitude for the exercise of independent professional judgment and initiative, serves as a Head Nurse in the Bureau of Medical and Professional Review (BMPR) of Home Care Services Program. As the Head Nurse, they will collaborate with other professionals (i.e., physicians, social workers, etc., assigned to a multi-disciplined Medical Review Team) in the systematic assessment of referrals for HCSP and the determination of appropriate types and amounts of service(s) required to promote clients’ optimal level of functioning and health. Moreover, will act as consultant to related CASA staff, as well as contracted service providers with respect to issues or questions involving nursing care standards, functions, directives and/or techniques.

Home Care Services Program (HCSP) is recruiting for one (1) Head Nurse, to function as a Head Nurse in HCSP/ CASA South office, who will:

  • Be responsible for review and decisions on all referrals (initial and reauthorizations) for home care services.
  • Use established principles of professional nursing to participate in systematic assessment of Medicaid client’s home care referrals and the determination of appropriate types and levels of home care services to be authorized or reauthorized.
  • Review medical documentation (i.e., diagnoses, prognoses, prescriptions, medical doctor’s orders, etc.) to determine the level and kind of home care required; make professional recommendations accordingly.
  • Keep abreast of medical and utilization review techniques and developments relative to the nursing profession for making recommendations regarding the program’s operation.
  • Serve as the intra-office specialist with responsibility for consulting and/or conferring with professional staff regarding all nursing aspects (i.e., needs, problems, activities, etc.) of the program.
  • Initiate and establish liaison with Certified Home Health Care Agencies (CHHCA) as required for the resolution of problems or differences of opinion between the division’s staff and the home health care agencies.
  • Participate in the formulation or modification of the home care standards or procedures, make recommendations to improve or enhance the delivery of home health care services.
  • Participate in the development of training materials.

Salary Range: $109,491 - $109,491.
Hours/Schedule: 9AM – 5:30PM (1 HR FLEX)

MINIMUM QUALIFICATIONS

  1. A valid New York State Registered Nurse License and completion of one year of satisfactory experience as a Registered Nurse.
  2. Education and/or experience equivalent to question “1” above. However, all candidates must have a valid New York State Registered Nurse License.

  • Demonstrated leadership skills. - Able to communicate well both verbally and in writing. - Able to efficiently utilize computer programs and enter data in an electronic field visit report. - Bilingual ability preferred. - Strong clinical assessment skills. - Exceptional Time Management skills. - Perform field visits, including visits to client homes, as necessary, to assess, reassess, or consult/resolve issues pertaining to recommendations for home care services. - Field and home visits will require travel within the five boroughs of New York City regardless of what HCSP site the Head Nurse is assigned to.
