We fulfill this mission through: • Improving access to primary health care for residents of South Bay • Employing skilled professionals committed to providing quality healthcare services and addressing current health needs • Serving the community with compassion and respect while maintaining confidentiality • Collaborating with other health care providers and organizations • Providing a medical home to the "at-risk" population GENERAL DESCRIPTION: Staff Clinical Registered Nurses (RN) work within a patient-centered team-based care environment to provide excellent patient care services within scope of practice as established by the Nurse Board of California. RN are members of patient-centered care teams. RN create access to care for patients by determining acuity of patient care when access is limited by providing excellent communication with patients. RN develop and maintain relationship, communication, and nursing skills with the overall goal of providing excellent patient-centered treatment, care, and services in a team environment. Services including routine primary care at a FQHC may include but not limited to adult, pediatric, gynecology, prenatal, and behavioral health. This role reports to the Director of Nursing. DUTIES AND RESPONSIBILITES: Staff Clinical Registered Nurses are responsible for knowing their legal scope of practice as defined by the Registered Nurse Practice Act and as delineated in this policy. The clinic RN is responsible for maintaining clinical competence with regards to these skills and activities expected of a licensed nurse. IBCC will support the registered nurse’s educational needs with regard to new skills or maintenance of skills and knowledge base in any required activity. A nurse should not perform an activity when the nurse does not feel clinically competent or questions the legal scope of practice for that activity. Questions should be directed to the nurses’ immediate supervisor for clarification. 1. Adheres to the IBCC Mission, Vision, and Values, Standards of Conduct and HIPAA principles. 2. Coordinates duties with and from licensed providers, Director of Nursing and Medical Director and Chief Executive Officer. 3. Consistently adheres to and/or exceed IBCC’s communication guidelines and expectations with patients, peers, and supervisors. · Communicates effectively by using welcoming words, proper tone of voice appropriate body language, eye contact. Greets every patient with a verbal greeting, eye contact and a smile. · Listens skillfully and displays a willingness and ability to acknowledge patients and others’ needs, expectations, and values using reflective listening and empathy conveyance. · Responds to patients and others needs in ways that are helpful and beyond expectation. · Builds positive relationships with patients and other IBCC staff to maximize accessibility of care to all patients. 4. Demonstrates excellent teamwork skills. Works collaboratively with other personnel to process and complete the patient encounter. Communicates effectively with care team throughout the day 5. Work with care team to actively manage the patient schedule to optimize efficiency and patient experience. Attends and contributes to departmental team huddle 6. Work collaboratively with Care Team to optimize communication, coordinate needed services and meet the needs of the patient. i.e. Lab/ BH/referrals/Outreach 7. Care coordination and/or case management for continuity of care (i.e. ER/Hospital) co-visits in collaboration with providers 8. Collects and records patient data for including but not limited to vitals and basic medical history. Records results of treatment and responses to care in the medical record, consistent with IBCC documentation protocols and standing orders or provider written/verbal instructions 9. Administers medications and vaccines as directed by licensed doctor, NP/CNM or standing orders 10. Collecting and preserving specimen, non- invasive technique 11. Performs emergency first aid and BLS/CPR. Manage/ support staff with clinic emergencies 12. Nursing management of but not limited to foley catheters, colostomies, gastrostomy tubes, wound care 13. Execute minor procedures i.e. staple and suture removals 14. Provides patient information instructions and education may include but not limited to: chronic disease management, cancer screenings, family planning and immunization 15. Assist patient and provider with medication refills per protocol, calls to pharmacies for routine, new medication refills and/or medication clarifications, obtains medication information and reports back to licensed provider and other items as needed. 16. Triage Lab results and manage abnormal lab results with providers. 17. Provide proactive utilization management for patients with acute high-risk needs, (i.e seniors on multiple meds) 18. Prevent inappropriate utilization of emergency room services 19. Oversee Care Team Medical Assistant administration of immunizations and medications, both injection and oral within the office visit. 20. Assist with patient transfers to/from emergency room 21. Provide Nurse Visits for designated conditions per clinical protocol (i.e. walk in pregnancy test) 22. Participate in activities related to addressing the social determinates of a patient’s health. 23. Actively seek out opportunities to assist other team members in collaboratively meeting the needs of the patient and care team. 24. Assist in the adoption and implementation of new technologies and activities that improve patient care and clinic efficiencies. 25. Phone Triage and Nurse Care Management. 26. Other duties as assigned by the Director of Nursing. Additional Duties and Responsibilities: 1. Participates in continuous quality improvement activities and patient-centered medical home team meetings and trainings, and is receptive to accepting other job duties as assigned and as delegated 2. Assists in responding to and resolving patient complaints as needed or directing patients to appropriate staff. 3. Participates in staff education and training activities in communication, care coordination, population management and program evaluation, as required 4. Enhances professional growth and development through participation in IBCC staff meetings, education and training activities for clinical staff, educational programs, in-services, meetings, and /or workshops. Maintains required competencies as established. 5. Maintains facility management by abiding by quality assurance, safety, environmental, infection control standards including following infection disease precautions to prevent the spread of organisms. 6. Point of contact for MA staff /other departmental questions regarding nursing care and/or CLIA Waived tests Assist with report submission as dictated Reports any clinical concerns or safety issues to clinic managers or directors 9. Performs other related duties as assigned or requested by nurse director or leadership team members QUALIFICATIONS: Education: 1. Bachelor's degree from four-year college or university preferred 2. Graduate of an accredited school of nursing. Certification/Licensure: 1. Maintain current State of California nurse licensure 2. Maintain CPR certification, PPD and other vaccinations Experience: 1. Minimum two years’ experience and/or training; or equivalent combination of education and experience. 2. Experience in FQHC preferred. Skills: 1. Excellent organizational and interpersonal communication skills. 2. Demonstrated ability to manage multiple priorities. 3. Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. 4. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and physicians. Other Requirements: 1. Must convey a professional demeanor. 2. Must be able to act calmly and effectively in a busy situation. 3. Ability to communicate effectively in the English language in person, by phone and in writing. 4. Requires adherence to all policies and procedures, including standards for safety, attendance, punctuality and personal appearance. 5. Must be able to establish and maintain effective working relationships with managers and peers. 6. Spanish translation is encouraged Physical Requirements: Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly in person and over the telephone. Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. Other: Requires frequent bending, reaching, standing, walking, squatting and sitting, pushing and pulling exerted regularly throughout a regular work shift.
