About This Role: * CLINICAL CARE & PATIENT SUPPORT * Provide direct patient care, including nurse visits, health education, and support for patients and families. * Document accurately in the EHR and ensure timely completion of clinical tasks. * Monitor patient portal activity and respond to inquiries as appropriate. * CLINICAL OPERATIONS & OVERSIGHT * Oversee EHR workflows and ensure completion of MA tasks and open order tracking. * Maintain lab compliance (CLIA-waived testing), medication inventory, and vaccine management (storage, logs, and monitoring). * Order and manage clinical supplies based on site needs. * QUALITY, COMPLIANCE & COLLABORATION * Ensure adherence to Evara Health policies and all HIPAA, State, Federal, and accreditation standards. * Partner with Leadership to support operations, team performance, and continuous improvement.
