Supporting both young people and adults, MHJHADS provides services in a variety of settings, including inpatient and outpatient facilities, community health centres, justice health environments, and within people’s homes. The service is committed to fostering social inclusion and operates within recovery-focused and harm minimisation frameworks. Adult Community Mental Health Services (ACMHS) teams work across the ACT, guided by established models of care tailored to both adult and older persons’ mental health. These teams respond to diverse community needs through a wide range of programs, including intake, recovery-oriented services, supported and therapeutic accommodation, specialised therapies, neuropsychology, assertive and homelessness outreach, and intensive home-based treatment. This role is based within the Assertive Community Outreach Service (ACOS), a multidisciplinary team delivering flexible, proactive, and community-based care to individuals experiencing severe and persistent mental illness, including psychotic disorders and severe bipolar disorder. ACOS supports people who may have found it difficult to engage with traditional services, providing consistent, assertive follow-up to improve outcomes, reduce relapse, and address complex challenges such as significant functional impairment, homelessness, and risk of suicide. Operating seven days a week from 8:30am to 9:00pm, ACOS provides extended hours coverage to ensure continuity and responsiveness of care, with this position participating in a rotating shift roster. The role offers the opportunity to work collaboratively within a supportive team environment, providing clinical management, including care for consumers receiving clozapine, while engaging closely with families, carers, and community partners. Based at the City Community Health Centre, this position combines team-based care with meaningful outreach work across the ACT, offering a unique and fulfilling opportunity to make a genuine difference in the community.
