Rehabilitation, Aged and Community Services (RACS) is seeking passionate clinicians to join the Community Care Program (CCP), a dynamic service delivering high‑quality nursing and allied health care across clinics, homes and community settings. In this role, you’ll help people build independence, provide expert clinical care that keeps them well at home, and work closely with GPs and other providers to support meaningful health and wellbeing goals. You’ll guide clients and carers through their journey between home and healthcare while delivering services such as wound care, post‑acute support, CVAD management, continence and stoma education, primary palliative care and end‑of‑life support.
