Within RACS, the Community Care Program (CCP) is a key service made up of Nursing and Allied Health clinicians supported by dedicated administrative teams, providing care either in clinic settings or in clients’ homes based on individual needs. In this role, you will work closely with people and their carers to build skills and confidence, supporting independence and self-management. You will provide clinical care in the community to optimise health outcomes and reduce unnecessary hospital admissions, while collaborating with general practitioners and other providers to achieve shared health and wellbeing goals. You will also help guide individuals through transitions between home and healthcare services, ensuring continuity of care. The service offers a broad scope of practice including wound care, post-acute care, CVAD management, continence and stoma support, as well as primary palliative and end-of-life care, making this a rewarding opportunity to make a meaningful difference in people’s lives.
