The Home-Based Palliative Care (HBPC) team delivers expert, holistic care to patients with life limiting illnesses who require specialist symptom management, end of life care, and psychosocial support in their place of residence. Care is delivered in partnership with patients, families, carers, and the wider healthcare system to optimise comfort, dignity, and quality of life. The Registered Nurse (Level 2) is a key member of the multidisciplinary team and is responsible for providing high quality, evidence based, and person-centered nursing care in the community setting. The role requires a high level of clinical competence, critical thinking, autonomy, and effective collaboration across professional and service boundaries.
