Join the Older Persons Mental Health Community Team (OPMHCT) and make a meaningful impact on the lives of older adults in the ACT. This specialist role offers the opportunity to provide expert, community-based assessment, treatment, and care planning for those experiencing or at risk of mental illness, whether living at home or in Residential Aged Care Facilities.
As part of the Adult Community Mental Health Services (ACMHS), you’ll work within a supportive, multidisciplinary team to deliver evidence-based, person-centred care. Your role will involve collaboration with carers, health professionals, and key stakeholders, as well as access to a range of programs including hospital diversion, assertive outreach, and community recovery services.
