Our multidisciplinary team provides the same high‑quality treatment patients would receive in an inpatient ward—just in a more comfortable setting. We are seeking a Clinical Care Coordinator (CCC) to provide senior clinical leadership and manage daily patient flow, including coordinating unit and home visits and handling internal and external referrals. As a Registered Nurse Level 3.1, the CCC also supports HITH nurses with clinical needs across the service. With growing demand, this role plays an important part in ongoing service development and works closely with the HITH Clinical Nurse Consultant (CNC).
