This center aims to provide a holistic and evidence-based approach to address the complex needs of families experiencing trauma and substance abuse disorder while focusing on delivering outcomes and recovery-based care. This facility will provide a supportive and healing environment for Individuals and families struggling with substance abuse issues. Our approach emphasizes a multi-disciplinary, person-centered, and family-focused model to address the diverse needs of those seeking recovery in Saskatchewan.

The Carter House will target women and families who:

• Must relinquish care of their children to access treatment.
• Are seeking treatment to regain custody of their children.
• Are at risk of having their children apprehended due to substance use.

JOB SUMMARY – REGISTERED NURSE MANAGER

The Registered Nurse will work with existing wrap around care and support offered by The Carter House through a harm reduction and trauma informed lens. The Registered Nurse will assist with coordination of medical care and follow up for both clients and infants related to but not limited to, medication adherence, managing MARs, fielding medical concerns, consulting with Pediatrics, Infectious Disease, facilitating medical appointments, child care, harm reduction, mood assessments and referrals to Primary Health Care.
The RN must have the ability to communicate and maintain effective working relationships with clients from diverse social, economic, and cultural backgrounds, and maintain an effective working relationship with other members of The Carter House team, as well as community health care providers and agencies.

Qualifications and Skills

• Bachelor Degree in Nursing
• Experience in Public health, pediatric care and or Mental Health and Addiction nursing is an asset.
• Maintenance of accurate client charts
• Strong knowledge of service providers within the areas we serve
• An understanding of the issues related to communicable disease, poverty, addiction, trauma and mental health
• Understanding of trauma informed care and its role in providing culturally sensitive and appropriate interventions for populations who have been exposed to trauma
• Understanding of harm reduction and its role in providing addictions treatment and health care
• Willingness to work with a team and independentl

• Create complete medical care plan for all parents and children of The Carter House
• Provide medical support to all Carter House clients including screening, testing, treatment and follow up
• Medication management and compliance: MAR reconciliation
• Liaison with primary health care sites and other health care professionals for parents and children of The Carter House for all care, transition planning, and follow-up
• Coordinate appropriate CBO and health agencies for full continuity of care
• Coordinate with Carter House interdisciplinary staff and community services when appropriate for full continuity of care
• Support triage of waitlist
• Support Continuing Care Assistants and Shift Supervisors with care planning and coordination as pertains to medical care
• Ensuring clear communication to Continuing Care Assistants, Shift Supervisors, and physician
• Preform toxicology screens, where necessary and follow up on toxicology screens sent to the lab
• Case conference and case plan when necessary
• Develop work standards for nursing position
• Attend appropriate medical appointments for continuity of care
