POSITION SUMMARY:

Coordinate documentation of MDS 3.0 and Care Planning through collaboration with all members of the Interdisciplinary Team. Is able to link MDS 3.0 and Resident Assessment Protocol information to the care planning process. Responsible for computer entry of all resident assessment data. Initiates and completes a comprehensive care plan for all residents according to current nursing policy and regulatory requirements.

QUALIFICATIONS:

· Current valid and unrestricted RN licensure in the state of Ohio.
· Current knowledge of gerontological nursing.
· Significant knowledge of licensure, certification, and other regulatory bodies of long-term care;
· Excellent clinical and assessment skills
· Ability to work independently and meet timeframes
· Relevant computer and software skills.

  • Is able to analyze complex patient data and make recommendations for individual patient care and facility policies.
  • Must maintain confidentiality of records and general information available through office records.
  • Must be able to speak the English language in an understandable manner.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately in order to fulfill the requirements of the position.

Responsibilities
  1. Coordinates documentation of MDS 3.0 and Care Planning through collaboration with all members of the Interdisciplinary Team.
    a. Utilizes the State of Ohio Resident Assessment Instruments to assess newly admitted residents and those residents who experience a significant change in status.
    b. Reviews and coordinates resident information from all interdisciplinary assessment forms, staff communications, resident/family interviews, and the medical record to complete the MDS 3.0 and the Resident Assessment Protocols.
    c. Reviews resident data and determines whether sufficient clinical information has been collected and documented. Involves appropriate clinical disciplines as necessary.
  2. Is able to link MDS 3.0 and Resident Assessment Protocol information to the care planning process:
    a. Links the MDS 3.0 and the triggered CAA’s to complete a CAA summary while assessing the need to proceed to the interim/comprehensive plan of care.
    b. Serves as a resource to facility staff in the completion of the MDS, CAA’s and plans of care.
    c. Documents according to the State and Federal regulations governing Resident Assessments.
    d. Provides in-service education to facility staff relative to documentation of necessary clinical information in order to substantiate MDS 3.0 assessment.
  3. Is responsible for computer entry all resident assessment data:
    a. Accurately completes and enters all resident assessment data into the computer program in a timely fashion in accordance with State and Federal guidelines.
    b. Adheres to final lock dates and submission requirements as stipulated by HCFA and State Standards.
    c. Maintains current working knowledge of MDS 3.0, RAP’s, and PPS requirements.
    d. Works collaboratively with MDS coordinator to ensure computer data entry remains as error free as possible; corrects any data entry errors in a timely manner.
    e. Works collaboratively with Administrative Services staff to “hand-off” accurate and timely MDS information for billing accuracy.
    f. Maximizes case mix score to achieve highest possible legitimate financial reimbursement for the facility.
    g. Assists MDS coordinator with submission of assessments according to Federal and State guidelines and with the Corrections Process as required.
  4. Initiates and completes comprehensive care plan for all residents:
    a. Coordinates and assists in developing a comprehensive plan of care in conjunction with the Interdisciplinary team for each resident within specific guidelines.
    b. Coordinates the Patient Care Conference in conjunction with the Interdisciplinary team to outline resident challenges and seek alternatives that foster the resident’s quality of life.
    c. Participates in quality improvement activities, site review, and conducts medical record reviews, as necessary.
  5. Attends and participates in continuing education programs and mandatory in-services.
  6. Conducts self with professional decorum at all times and with all persons in the facility.
  7. Must be able to remain calm under stressful situations and tactfully deal with all personnel, residents, visitors, and government agencies.
  8. Follows established Infection Control and Universal precautions policies and procedures.
  9. Knocks before entering the resident’s room.
  10. Assures that the resident’s rights are observed as they pertain to the position.
  11. Demonstrates respect for resident’s and families.
  12. Maintains the confidentiality of all resident care information.
  13. Encourages resident participation in planning and implementing nursing care.
  14. Ensure that all residents are treated fairly, and with kindness, dignity and respect.
  15. Reports all unsafe/hazardous conditions, defective equipment, etc., to appropriate department immediately.

  16. May be asked to perform other duties as deemed necessary by MDS Coordinator.
