We are expanding our scope of services and looking for a confident practicing Registered Nurse to join our Vancouver office as our lead Client Care Manager!
Responsibilities:
- Initiate and manage medication administration and/or nursing delegated specialized services to clients.
- Maintain a current plan of care outlining the specific specialized services to be provided to the client.
- Ensure proper maintenance of client journals: collect service records for office file and keep journal stocked with appropriate documentation forms.
- Provide oversight, up to and including the instruction of initial and ongoing in-office and field training for CAREGivers.
- Supervise ongoing case management of all patients ensuring an accurate evaluation and treatment plan, maintaining our quality model, and utilizing services appropriately to achieve optimal outcomes.
- Communicate with clients and families as needed regarding satisfaction and/or issues of clients.
- Maintains ongoing communication regarding patient care with physicians, referral sources, caregivers, patients, and associates to facilitate coordination of care. Assure physician approval of plans for continuity of care. Ability to communicate with patients of various levels of functioning and various types of personalities.
- Develops relationships with referral partners, promoting the company and supporting sales and marketing initiatives
- Develop short and long-term goals for the entire care department.
- Establish new policies and update existing policies to improve the standard of care for clients.
- Respond to any nursing-related issues in a timely manner.
- Maintains effective and efficient ongoing communication with the senior management related to patient care, physician relations, community relations, or other areas impacting agency quality performance.
- Provides performance feedback and coaching to direct report team members.
- Adhere to established confidentiality standards and privacy guidelines concerning community and patient information.
- Consistently demonstrates sound judgment in the day-to-day interactions with co-workers, management staff, and clients.
- Maintain positive relationships and rapport with co-workers and community associates.
- Supervise and participate in emergency on-call support.
- Conduct ongoing in-office and field trainings for CAREGivers.
- Conduct specialized services trainings with individual CAREGivers who are serving high needs clients receiving specialized services. The training must include a return demonstration performed by the CAREGiver and signed off/dated by the nurse. This documentation must be retained in the CAREGiver’s employment file.
- Act as an expert adviser regarding the medication services and delegated specialized services training given to CAREGiver.
- Keep detailed documentation of all relevant phone and in-person contact with clients, client representatives, CAREGivers and others.
