Make your own schedule | Mileage Reimbursement | Part-Time Responsibilities: Completing the nursing process in the delivery of patient care, which includes assessment, planning, implementation, and evaluation of patient outcomes. Total oversight of patient from admission to discharge. Developing a plan of care based on findings. Serving as patient’s advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care. Timely completion of all skilled nursing documentation. Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs) Multidisciplinary team approach (coordinating with all disciplines of patient care, including PT, OT, ST, MSW, HHA's, and office staff) Requirements Current Illinois State RN. CPR Certification Previous experience in nursing or other health field experience Familiarity with medical software and equipment Ability to build rapport with patients Strong problem-solving and critical-thinking skills Ability to thrive in a fast-paced environment WellSky EMR knowledge is preferred but not required. Compensation: $65-$75/per visit, commensurate with experience
