This includes conducting patient admissions, developing care plans, and coordinating care with the interdisciplinary team. Key Responsibilities: • Obtain medical history and assess patient condition and needs • Conduct physical assessments, including vitals and mobility evaluations • Educate patients and families on care plans and home health services • Deliver skilled nursing care while maintaining HIPAA compliance • Document and submit accurate visit records promptly • Develop and update individualized care plans as needed • Coordinate with LPNs, Home Health Aides, and Personal Care Attendants • Facilitate referrals for additional services such as therapy or medical social work • Evaluate the effectiveness of nursing services and adjust as appropriate Qualifications Qualifications: • Active RN license in NV • Experience with OASIS and WellSky (Kinnser) strongly preferred • Med-surg and wound care experience a plus • Current physical exam (within last 12 months) • Current TB test (within last 12 months) and CPR certification • Valid driver’s license • Must be able to attend orientation at our Las Vegas location Where We Serve: We offer opportunities in AZ, CA, CT, FL, IL, IN, MA, MN, NV, and OH.
