Position Overview Location: Patient visits across San Diego Schedule: Full-Time Compensation: $55–$62/hour (commensurate with experience) This role is community-based, with visits to patients in their homes, assisted living communities, and skilled nursing facilities. As part of our interdisciplinary team, you’ll deliver holistic, patient-centered care that supports both physical comfort and emotional well-being. Key Responsibilities Assess patient needs, triage priorities, and deliver skilled nursing care Collaborate with providers, social workers, and spiritual counselors to implement individualized care plans Support emotional well-being and legacy-building activities for patients and families Introduce and coordinate complementary therapies (e.g., massage, music therapy, Reiki, journaling) Facilitate advance care planning conversations and assist with directives Provide education on symptom management and coping strategies to patients, families, and staff Ensure continuity of care across settings and team rotations Document care accurately within RN scope of practice Serve as a resource for clinical teams on palliative care principles Qualifications Active, unrestricted RN license in California Minimum of 2 years of nursing experience (palliative/hospice experience preferred) Associate’s or Bachelor’s degree in Nursing Community-based or home health experience a plus Valid driver’s license, reliable transportation, and current auto insurance required Why Join Us?
