Description This is for a PM Shift. Through a holistic approach, high quality comprehensive care is provided timely and efficiently to promote a positive end of life experience in the PCU. Requirements ESSENTIAL DUTIES & RESPONSIBILITIES: · Conduct admission assessments · Establish plan of care in coordination with interdisciplinary team approach to collaborate, revise, update, and assure services are provided according to established plan · Make assignments and supervise patient care provided by volunteers and staff members · Ensure physicians, families, and appropriate staff members are promptly notified of changes in patient condition · Make referrals to other services as needed in coordination with social workers · Educate families and patients on disease processes, dying process, treatment plan and assist them in making informed choices about care promoting independence · Provide active listening and support to patients and families through the trajectory of their disease progression · Provide services at the time of death, including notification of staff per policy · Provide medications timely and accurately per physician’s orders · Ensure safety of all residents implementing interventions needed as per plan of care · Ensure infection control policies and procedures are implemented and followed · Assure facility is secured per safety policies and procedures after hours and on weekends · Assure facility environment is appropriate to promote privacy and solace for all patients · Assure patient/facility laundry is appropriately cleaned and restocked · Other duties as assigned KNOWLEDGE, SKILLS & ABILITIES: · Able to work with the interdisciplinary team · Willingness to work with patients/families regardless of race, color, creed, age, or sexual orientation. Must respect their beliefs and philosophy with an attitude of openness and receptiveness MINIMUM QUALIFICATIONS: Education: § Current North Carolina license as a registered nurse Experience: § Minimum 2 years nursing experience preferred Licensures & Certifications: (if applicable): § CPR certification § CHPN within 3 years of hire
Responsibilities
