Act as the central communication bridge connecting management, physicians, insurers, and community partners to ensure seamless collaboration. Align treatment efforts across teams while actively driving quality improvement initiatives and educational programs. Track patient outcomes, compile key metrics, and pitch smart recommendations to continuously improve clinical processes. Bridge the gap for uninsured and underinsured patients by connecting them with essential follow-up care and community resources. What You Will Need Licensure / Certification / Registration: Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time ofhire.
