How you'll make an impact in this role Serve as a dedicated patient navigator throughout the continuum of care, ensuring individuals experience seamless, well-coordinated transitions and feel supported at every step of their health journey. Function as a central liaison between management, physicians, managed care companies, and community organizations to streamline communication and deliver a unified care experience. Coordinate strategic treatment efforts while actively promoting quality improvement initiatives and educational programs that elevate overall standards of care. Compile and analyze key clinical data and outcome tracking to drive meaningful process improvements across the department. Provide vital support and resource referrals for uninsured or underinsured patients, bridging the gap to ensure everyone has access to essential follow-up care. What minimum qualifications you'll need Licensure / Certification / Registration: Licensed Registered Nurse credentialed from the Kansas Board of Nursing required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Education: Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
