Supervises, teaches, and delegates to other personnel in the performance of activities related to nursing care. * Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. * Ability to formulate an individualized plan of care, revise plan as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness. * Monitors, records, and communicate patient condition as appropriate to care team, provider, patient and family following chain of command as appropriate. * Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs, * Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition and circumstances. * Works as an advocate for the physical and emotional well-being of the patient protecting the patient's rights   Why Join Us: We believe that investing in our employees is the first step to providing excellent patient care.
