Your responsibilities include: • Developing and maintaining individualized client care plans • Completing supervisory visits and in home assessments • Monitoring client condition and recommending care adjustments • Providing hands on caregiver training  • Ensuring caregiver competency with delegated tasks • Reassessing clients every 90 days or sooner if needed • Documenting changes in condition and updating care plans • Communicating clearly with clients, families, and the care team     Whether you’re stepping into your first RN role, starting your nursing journey as a new grad, or you’re an experienced clinician looking for better balance and autonomy, this opportunity offers meaningful work with support every step of the way.  
Responsibilities
The Registered Nurse will develop and maintain individualized client care plans while conducting regular in-home assessments and supervisory visits.
