Job Description: · Fitzroy location, Part time 0.8EFT, fixed term until 30th June 2027 · Challenging and rewarding work environment · Opportunities for career development About the Role The primary aim of the position is to provide clinical assessment and implement clinical management of residents of Residential Aged Care Facilities (RACF’s) and the community in order to prevent avoidable presentations to the Emergency Department (ED). The position also provides clinical consultancy to nursing staff and care staff of RACF’s and relevant community agencies. The duties and responsibilities associated with this position include but are not limited to the following: · Provide comprehensive assessments, care planning, coordination and treatment for RACF and community residents who are experiencing medical issues which may lead to an emergency department presentation. · Provide clinical advice to RACF staff, patients, carers and other health care professionals within the defined speciality and evaluate treatment outcomes using appropriate measures · Actively support the timely discharge of RACF residents from within ED/inpatient units and ensure follow up to minimise future representations where appropriate. The RIR service works closely with hospital based teams and services, Residential Aged Care Facilities (RACFs), General Practitioners (GPs), and other community health care teams to assist in the ongoing coordination and delivery of timely and effective care for older persons by providing early intervention in relation to acute illnesses associated with chronic disease and ageing, and providing evidence-based education and nursing leadership. This service is nurse led and is staffed by a Nurse Practitioner Candidates and Clinical Nurse Consultants and rotating medical staff from the Geriatric Medical Unit.
