Description

The Registered Nurses will play a key role within the multi-disciplinary team, providing high-quality, person-centred nursing care and support to the young people in the residential school setting. Working in collaboration with care staff, managers, and other professionals, the nurse will contribute to a safe and nurturing environment that promotes the health, well-being, and development of all young people. The primary objective is to ensure that the health needs of the young people are met, enabling them to achieve their personal outcomes. The young people supported at St Philip’s School may present with complex behavioural difficulties, often linked to previous trauma and neurodevelopmental factors such as Attention Deficit Hyperactivity Disorder (ADHD), Autism Spectrum Disorder (ASD), and Foetal Alcohol Spectrum Disorder (FASD).
This role requires the job holder to work a flexible shift pattern, 5 over 7. Shifts will include a combination of early, back, day shifts, twilight and nightshifts as to meet the needs of the service and the young people.

Responsibilities
  • Clinical Care and Assessment:
  • Conduct comprehensive health assessments and develop, implement, and evaluate individualised care plans.
  • Provide direct nursing care, including medication administration, wound care, and management of various health conditions (e.g., epilepsy, diabetes, etc.).
  • Monitor and manage the physical, psychological, and social health needs of the young people, recognising and responding to changes in their condition.
  • Act as the lead for all clinical matters, ensuring the delivery of a safe and high-quality service.
  • Multi-Disciplinary Team Collaboration:
  • Work closely with residential care staff and managers, providing clinical guidance, support, and training on health-related matters.
  • Liaise with external health professionals such as GPs, hospital teams, and community nurses to ensure seamless continuity of care. This will involve engaging with services and teams within the Lanarkshire Health and Social Care Partnership, including Child & Adolescent Mental Health Services (CAMHS), social work, and other specialist community teams.
  • Actively participate in multi-disciplinary meetings, contributing to the development of young people’s care plans and outcomes.
  • Safety and Safeguarding:
  • Ensure all nursing care is delivered in accordance with the Nursing and Midwifery Council (NMC) Code and all relevant legislation and professional guidelines.
  • Uphold the highest standards of clinical governance and maintain accurate, up-to-date health records.
  • Be aware of and adhere to all safeguarding policies and procedures, reporting any concerns promptly.
  • Leadership and Training:
  • Lead and support the wider care team in the delivery of health-related tasks, including delegation of duties where appropriate.
  • Provide training and support to residential care staff on a range of health topics, such as emergency procedures, medication management, and recognising signs of illness.
  • Act as an advocate for the young people, ensuring their health needs and rights are at the forefront of their care.
