Brighten Life Home Health is looking for an On-call Registered Nurse for Home Health in St George!   Schedule: 7 on and 7 off On Call or Just On-Call $38.00 - $42.00 per hour $50 a night Monday - Thursday, plus hour rate $250 a weekend Friday- Sunday, plus hour rate   Brighten Life Home Health is growing and excelling. We specialize in caring for Veterans, but we service all peoples.   Apply today for a fulfilling and worthwhile career with Brighton Veterans Home Health!   Responsibilities to include: admissions, catheter changes, medication needs, falls, urgent needs that need to be addressed and documenting all care performed in accordance with the plan of care.     Responsibilities * Completes an initial assessment of patient and family to determine home care needs. Including a complete physical assessment and history of current and previous illness(es). * Regularly re-evaluates patient nursing needs. * Initiates the plan of care and makes necessary revisions as patient status and needs change. * Uses health assessment data to determine nursing diagnosis. * Develops a care plan that establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. * Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. * Counsels the patient and family in meeting nursing and related needs. * Provides health care instructions to the patient as appropriate per assessment and plan. * Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. * Participates in on-call duties as defined by the on-call policy. * Ensures that arrangements for equipment and other necessary items and services. * Instructs, supervises and evaluates home health aide care provided every two (2) weeks * Prepares clinical notes and updates the primary physician when needed and every 60 days. * Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required. * Communicates with community health related persons to coordinate the care plan.   Qualifications * Must be a Registered Nurse in good standing with the State of Utah. Maintains a current license throughout employment. * Home Health Experience Preferred. * Physically able to lift, turn, position and transfer patients. * Have reliable means of transportation, current auto insurance and driver’s license. * Demonstrates working knowledge of infection control principles and practices.    
Responsibilities
Responsibilities include completing initial and regular patient/family assessments, developing and revising the plan of care, administering prescribed treatments, and counseling patients on their needs. The role also requires documenting all care, participating in on-call duties, and coordinating with physicians and community health personnel.
