Position Overview Department: Case ManagementSchedule: Full Time Days, Monday - Friday 8:00AM - 4:30PMWork Location: UAB St. Vincent's East Benefits for eligible positions only include: generous paid time off, paid parental leave, Associate Assistance Program, Tuition Reimbursement Program, and more What You Will Do Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge. Creates plan for care across the continuum, integrating patient/family preferences and values. Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records. Advocates for resources and removal of barriers. Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care. What You Will Need Licenses / Certifications / Registrations BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required. Education: Diploma from an accredited school/college of nursing and professional licensure at time of hire required. Additional Preferences No additional preferences.
