SUMMARY As a Community Health Nurse, you will focus on providing nursing interventions, patient education, and care management to a variety of clients in our community. Job Title: Licensed Practical Nurse (LPN) - Community Health Department: Home Care Location: Rochester Regional Health Home Care - 800 West Miller Street Suite 6 Newark, NY 14513 Hours Per Week: Per Diem Days/Evening SCHEDULE: Per Diem - As Needed ATTRIBUTES Compassionate, independent, and patient focused Comprehensive nursing assessments and documentation, decisive judgment and professional behavior Two (2) years' experience in home care preferred Bilingual preferred At least one year of nursing experience RESPONSIBILITIES Patient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care Planning. Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly; coordinates client services and/or referrals in an efficient and timely manner Documentation. Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards Compliance. Adhere to required department and system protocols, regulations (local, state, federal) and education requirements Minimum Qualifications: • Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred. Required Licensure / Certification Skills: • Registered Nurse license in New York State. • Valid NYS Driver’s License. EDUCATION: AS: Nursing (Required) LICENSES / CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. PAY RANGE: $75,734.00 - $101,014.00 CITY: Newark POSTAL CODE: 14513 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
