On Call are not eligible for any of the above except 401k **Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.**   Responsibilities * Provide direct nursing care to residents in accordance with care plans, facility policies, and regulatory guidelines. * Administer medications and treatments accurately and timely. * Monitor residents’ conditions and report significant changes to physicians and other healthcare team members. * Conduct assessments and document patient progress, symptoms, and vital signs. * Coordinate with interdisciplinary teams to develop and implement individualized care plans. * Supervise and support LPNs, CNAs, and other nursing staff to ensure high-quality resident care. * Respond promptly to emergencies and provide appropriate interventions. * Educate residents and families on health conditions, care plans, and post-discharge instructions. * Maintain accurate and timely clinical documentation in electronic medical records (EMR). * Ensure compliance with infection control protocols and health and safety regulations. * Promote dignity, independence, and overall well-being of residents. Qualifications * A current, active license to practice as a Registered Nurse (RN) in State of practice. * Demonstrate RN clinical assessment skills at the level necessary to meet the job requirements. * Recent clinical experience, education, or specialty skills specific to geriatrics. * Demonstrate understanding that state and Federal rules and regulations govern the practices within the facility. * Ongoing pursuit of Continuing Education Credits in clinical subjects, management, personal growth and development * Must pass criminal background investigation, reference inquiry, and vaccine mandate attestation requirements of either having been vaccinated or present documentation needed for a medical or religious exemption.   Avalon Health Care Group is an Equal Opportunity Employer.
